Hardcopy Form 2024 MOE FAS Application Form

Application strictly via hardcopy form is required for:

1. families with other dependents (Great grandparents, Aunties/Uncles, Cousins and others)
2. family with more than 6 children in government school
3. family with more than 5 unmarried children not in government school
4. family with more than 3 grandparents in the household

Note:

1. If your child is a ComCare recipient, you do not need to submit the application form for MOE FAS as MOE FAS will be auto granted to ComCare beneficiaries.
2. Please ready all softcopy supporting documents to be prepared in advance for eFAS application. 
3. Please submit the completed hardcopy FAS forms and supporting documents at the General Office.
